Withdrawal form template

Pursuant to Article 30 of the Polish Consumer Rights Act (consolidated text: Journal of Laws 2024, item 1061), this form may be used to withdraw from a distance contract. This form is also compliant with Article 11 of Directive 2011/83/EU.

Instructions for completing the form

1. How to submit the form

1. Complete the form - provide the order number, date of contract conclusion, and your personal details
2. Sign the form - required only for the paper version
3. Send the form - by email to: kontakt@tarabaseny.com.pl

2. Withdrawal period

• You have 14 days from the date of receiving the goods to withdraw from the contract
• The deadline is met if you send your withdrawal statement before it expires

3. Effects of withdrawal

• Withdrawal from the contract terminates all obligations under it
• The seller will reimburse all payments received without undue delay, and no later than 14 days from receipt of the withdrawal statement
• Reimbursement will be made using the same payment method you used, unless you explicitly agree to a different method

4. Return of goods

• You are obliged to return the goods without undue delay, and no later than 14 days from the date of withdrawal
• You bear the direct costs of returning the goods
• Goods must be returned to: Patryk Reksa Gold Frieza, ul. Kościelna 1, 62-030 Luboń, Poland
• The goods should be returned in an unchanged condition, unless the change was necessary due to normal use

Exceptions to the right of withdrawal

The right of withdrawal does not apply in the case of:

1. Perishable goods or goods with a short shelf life
2. Sealed goods - if the packaging has been opened after delivery (for health or hygiene protection reasons)
3. Goods that are inseparably combined with other items after delivery
